Percutaneous Epididymal Sperm Aspiration (PESA) PESA is a procedure performed for men who are having sperm retrieved for IVF/ICSI who have obstructive azoospermia from either a prior vasectomy or infection. When a patient suffers a blockage of the vas deferns, his sperm count needs to be examined through a process known as Percutaneous Epididymal Sperm Aspiration (PESA). The objective of the process is to find the sperm in the patient does not have sperm in his ejaculate, a condition known as Azoospermia. At our laboratory, this process is usually done after vasectomy and it enables the retrieval of thousands to millions of sperms under the high caliber guidance and direction of our experts.

Intrauterine Insemination (IUI) Intrauterine insemination or IUI is a basic treatment for infertility where washed sperm is placed directly into the uterus using a catheter around the ovulation time. Decreased sperm motility or low sperm count in males whereas cervical factor, endometriosis, unexplained etc, are the commonest indications for IUI . Couples who have tried natural or Ovulation induction more than 5-6 times but in vain are recommended to step up to IUI. However, they are certain prerequisites and contraindications to this procedure for which you need to consult a fertility specialist. It is a less invasive procedure compared to IVF and the process of fertilisation to implantation is natural hence success rate is 15-20% which is a little higher than natural cycles but of course lower than IVF. It’s a pretty cost-effective procedure and can be tried in 4-6 cycles as advised by your fertility expert.

In Vitro Fertilisation (IVF) In vitro preparation (IVF Procedure) is an artificial reproductive technology wherein embryos are cultured in an artificial environment that mimics the womb using a specially designed laboratory. The process of IVF Treatment involves stimulation of ovaries using gonadotropin injections with the aim of harvesting maximal eggs: the extracted eggs are mixed with the sperms in the IVF laboratory. The fertilised egg which is called an embryo is transferred back to the woman’s womb for the further process of implantation and development. There are various treatment protocols depending on the patient’s condition and need. The process starts with an evaluation of both partners, preparing them with multivitamins and lifestyle modifications which includes psychological counselling. Depending upon the protocol, injections are administered from day 2 or 3 of menstrual cycle for approximately 10 to 12 days. Once the follicles are ready, egg retrieval is done under anesthesia using ultrasound guidance. The eggs are then evaluated for maturity and mature eggs are inseminated with sperms in a culture dish and observed for 3 to 5 days to Check and assess the fertilisation and cleavage process which are the stages of embryo development. IVF helps us to know and understand the potential of gametes to form embryos. After 3 to 5 days of Culture, the Embryos are then transferred back to the uterus. If for whatsoever reason embryos cannot be transferred in the same cycle, they are frozen and then transferred in the next cycle. This process is called frozen embryo transfer. It involves the preparation of the endometrium which is the lining of the uterus where the embryo implants and then placing the warmed embryos back into the uterus. The lady is administered progesterone support for the next 12-15 days after which the pregnancy test is advised. Ivf pregnancies usually need support medications for the first 3 months after which routine medicines only are continued. The process of implantation is still naturally driven and inspite of all the research and technology one cannot predict the implantation, thence keeping the success rates between 50-70%.

Testicular Sperm Aspiration Or Extraction (TESA) This a procedure where sperms are retrieved directly from the testis in contrast to the normal ejaculatory sample. TESE involves making a small incision in the testis and examining the tubules for the presence of sperm, whereas TESA is an aspiration of the sperm with a thin needle without an incision. These methods of sperm retrieval are done during ISCI procedures where a single sperm needs to be injected into an oocyte. TESE and TESA have made it possible for azoospermic patients to conceive with their own sperms. These procedures usually don’t carry major risk to the patients.

Intracytoplasmic Sperm Injection (ICSI) ICSI-IVF is an advanced ART procedure wherein a single good quality sperm is selected under a micromanipulator using injection needle and is injected directly into a single egg. The further process of fertilisation and cleavage remains similar to Ivf. There are some specific conditions where fertilising capacity of the sperm is doubtful either due to the thick egg capsule ( zona) or the sperm’s inadequate acrosome reaction. These conditions may be helpful by direct injection of sperm into the egg by ICSI. ICSI is usually adviced and advocated in cases of previous fertilisation failure, unexplained infertility, thick Zona , decreased sperm count and motility , poor sperm morphology , testicular sperm etc .

Intracytoplasmic Morphologically Selected Sperm Injection (IMSI) Preparation and selection of sperm for IMSI have previously been described [19]. An aliquot of the sperm preparation was placed in a glass-bottomed dish (WillCo-dish, WillCo Wells BV, Amsterdam, The Netherlands) and examined by the Nomarski interference contrast microscopy with a Leica DFC-280 camera (Leica Microsystems, Nanterre, France) mounted on a Leica DMI 6000 microscope with an immersion objective lens ×100 and camera magnification ×1. Spermatozoa with the smallest relative vacuole area were preferentially selected [6]. If available, spermatozoa without vacuoles or with only small vacuoles were preferentially injected.
